General Case Management Activities

VIP Case Managers perform a range of services within and across the broader need areas to assist clients and families in meeting identified recovery goals.

Perform cross-need case management support for client
  • Research and describe relevant programs and organizations for client/caregiver needs and goals
  • Remind client/caregiver of upcoming appointments
  • Support transportation
  • Accompany client/caregiver to relevant appointments and meetings
  • Validate emotions and provide psychoeducation around trauma/traumatic responses
  • Share information on behalf of client/caregiver with external systems
  • Inform client/caregiver of system/program expectations
  • Assess and plan for safety in accessing community services
  • Advocate on behalf of client/caregiver
