General Case Management Activities
VIP Case Managers perform a range of services within and across the broader need areas to assist clients and families in meeting identified recovery goals.
- Perform cross-need case management support for client
- Research and describe relevant programs and organizations for client/caregiver needs and goals
- Remind client/caregiver of upcoming appointments
- Support transportation
- Accompany client/caregiver to relevant appointments and meetings
- Validate emotions and provide psychoeducation around trauma/traumatic responses
- Share information on behalf of client/caregiver with external systems
- Inform client/caregiver of system/program expectations
- Assess and plan for safety in accessing community services
- Advocate on behalf of client/caregiver
