VIP connects violence-impacted youth to the appropriate medical care for their injury-related and on-going medical needs and well-being.
- Assist client/caregiver in developing comprehensive plan for complex medical needs
- Obtain recommendations and/or referrals from providers
- Call medical providers and coordinate care with medical team(s)
- Provide education to client/family regarding different components of medical care (e.g., PT, concussion clinic) to support physical recovery
- Coordinate with home care services to provide recovery support
- File VCAP claim for client medical expenses
- Outline application process
- Assist in completion of application
- Provide guidance about and assist with supplemental application components (e.g., receipts)
- Request itemized medical bills from provider to submit with application
- Ask questions of VCAP personnel on behalf of clients and caregivers
- Submit physical copy of VCAP application or provide necessary mailing supplies (envelope, stamps, etc.) to caregiver
- Follow-up on status of application and/or payment
- Assist client/caregiver in medical insurance application process
- Provide client/caregiver with insurance-related application
- Support completion of insurance-related application
- Refer client to specialty care provider
- Coordinate specialty care referral with PCP or other provider
- Discuss role of specialty care provider with client/ caregiver
- Schedule specialty care appointment
- Refer client to primary care provider (PCP)
- Research or locate potential PCPs
- Schedule appointment with PCP
